If you have five or more people
who wish to apply for their My Number Card,
we’ll visit your site
and offer free-of-charge on-site application assistance, from taking their ID photos to accepting their applications.
Service process
STEP1

Organize at least five people who wish to apply.
*If you have trouble getting out, you can use this service by yourself.
STEP2

Call the Call Center to schedule a visit.
STEP3

Prepare
the required documentation and wait for the visit.
STEP4

A staff member will visit your location to accept your applications.
(ID photos will be taken free of charge.)

Receive your card at home.
*If you have trouble getting out, you can use this service by yourself.
Use of this service
●Use of This Service
After organizing at least five people (or even a single person who would have difficulty getting out) wishing to apply for their My Number Card for the first time or to apply to renew their card, call the Call Center no later than five business days in advance to schedule your visit. (Applicants must be residents of the city of Osaka.)

- • Please provide indoor space and parking for one kei-size vehicle on the day of the appointment.

●Eligibility
• Residents of the City of Osaka who are applying for their My Number Card for the first time
• Residents of the City of Osaka who are applying to renew their My Number Card
- *We're unable to provide support for renewing electronic certificates. If you wish to renew your electronic certificate but have difficulty going out, please consult your local ward office counter.
- *Service is not available for people who are requesting issuance of a new card because their card was lost or stolen.
- *Minors under the age of 15 and adult wards must be accompanied by their legal representative (parent or adult guardian).

Facilities, offices, private homes, and other locations within the city of Osaka
*On-site service is only available within the city of Osaka.
●Information required when making an appointment
Desired time, date, and location; number of people using service; breakdown of people using service (number of people who have not yet applied, number of people who have already applied and number of people applying for renewal); and representative’s name and contact information
●If you have trouble getting out
Individuals who have trouble getting out, for example due to illness or disability, may utilize this service alone (without organizing a group of five or more). Family members living in the same household may use the service along with them.
Examples of people who have trouble getting out :
- • People confined to bed due to illness
- • People receiving nursing care who have trouble getting out
- • People who have trouble getting out due to a disability
●PIN numbers
To facilitate a smooth application process, applicants should choose PIN numbers for their My Number Card as described below ahead of time.
- • One PIN number consisting of 6 to 16 alphanumeric characters (for use as an electronic certificate for the bearer’s signature)
·Letters must be uppercase.
- • Three PIN numbers consisting of four digits (for use as an electronic certificate for user identification, for use with basic resident registration card, and to support entering items on the card)
*The same PIN number can be used for all three applications.
●About the Facial Recognition My Number Card
The Facial Recognition My Number Card restricts the identification process to hardware-based facial recognition or visual confirmation, eliminating the need for a PIN number.
If you’re interested in getting a Facial Recognition My Number Card, please let a staff member know on the day of your visit.

*We’re unable to accept your application if your card has been lost or if more than three months remain before the expiration date.
*After submitting your application, either you or an authorized representative must come to the ward office counter to collect the My Number Card.
*We're unable to provide support for renewing electronic certificates. If you wish to renew your electronic certificate but have difficulty going out, please consult your local ward office counter.
*Please be aware that there will be a period during which you will not have your My Number Card in your possession.

- *A My Number Card that has exceeded its expiration date can only be used as an identity verification document by the applicant if no more than six months have passed since the expiration date.
- *If you can’t bring (1) and (3) with you, you’ll need to visit your ward office with identification documents to collect your card.
- *You won’t be able to use the My Number Card installed on your iPhone to verify your identity.

On the day of the visit
On the day of the visit, you’ll complete
your My Number Card application as described below.
Please note that if you don’t have
all the required documentation,
the steps in the process may differ,
or you may not be able to utilize the service.
STEP1

Have your photo ID taken.
STEP2

Fill in the application.
STEP3

Set your PIN numbers, etc.
STEP4

Present your ID.

The entire process, from taking your photo ID to
completing the application, takes about 20 minutes.
- *Additional time is required for set-up.
- *The staff member will collect applications, otification Cards being returned to the city, and other documentation before leaving.

Once your application is complete, all you have to do is wait for your My Number Card to arrive.
*It should arrive about one month after you submit your application.
The health insurance card expired
on December 1, 2025.
On-site Application Service can also help you apply for a health insurance card
and other documents.
Please take advantage of this service
if you have a My Number Card
but haven’t yet registered.

- • The On-site Application Service cannot be used solely to apply to use your My Number Card as your health insurance card.
Please be sure to apply along with someone who's applying for a My Number Card. - • We’re unable to provide assistance if you have a Facial Recognition My Number Card.
If you have a Facial Recognition My Number Card, please visit a nearby medical institution, pharmacy, or other healthcare provider.
